What Is Mobile Primary Care?

Mobile Primary care provides physician services for the elderly and other adults with complex health issues who have difficulty getting out to the doctor’s office.

We offer many in-home services including primary care in addition to servicing patients is Skilled Nursing facilities, Assisted Living facilities and patients with developmental disabilities.

We are dedicated to providing patients with high quality, personalized and confidential medical care.

Who Is It For?

Mobile Primary care is specifically targeted towards home-bound and mobility-limited patients.
These patients may reside in:

• Group Homes

These patients are generally elderly and require advanced care.
Elderly and home-limited patients often do not have the transportation or family resources to access the Primary Care they require.
